CMEDS Hours of Operation

CMEDS operates Monday to Friday from 8:30am-4:30pm. AHP Medical Benefits adjudicators are available to receive equipment requests and send them to CMEDS for processing during these hours of operation. CMEDS does not operate on the weekends, and the program does not offer emergency after hours or weekend support. Please note the program is also closed for all Canadian Statutory holidays.

Loan Request Form

An Occupational Therapist or Physiotherapist recommendation is required for all mobility and adaptive equipment. Therapists must complete the CMEDS Equipment Loan Request form and submit the request to AHP Medical Benefits for review. If approved, the request is forwarded to CMEDS to be processed. CMEDS will advise the therapist and AHP Medical Benefits if the requested equipment is available for loan. If suitable equipment is not available through CMEDS, AHP Medical Benefits may consider funding the medically necessary equipment.

Please see the CMEDS Equipment Loan Request Form. Please ensure that the Loan Request Form is filled out completely and all details are provided, including the child or youth's date of birth and current address.
